LEVEL 2

Includes 2 hours of review, 2 hours of drafting, and a 1-hour consultation call with Lemasters to discuss what changes were made and why.
This package is usually good for 2-3 forms/documents or approximately 6 pages. 

LEVEL 3

Includes 5 hours of review, 5 hours of drafting, and a 90 minute consultation call with Lemasters to discuss what changes were made and why.
This package is usually good for 5-6 forms/documents or approximately 15-25 pages.

LEVEL 4

Includes 10 hours of review, 10 hours of drafting, and a 90 minute consultation call with Lemasters to discuss what changes were made and why.
This package is usually good for 10-15 forms documents or approximately 30—50 pages.

CREMATION REVIEW PACKAGE

This package is for any deathcare provider who offers cremation. Lemasters will take a comprehensive look at your existing forms and help you to add new ones to your process. This includes Cremation Authorization, Cremation Checklist, Receipt of Human Remains, Receipt of Cremated Remains, Cremation Certificate, Crematory Log, Personal Affects Inventory, Identification without Viewing, Cremation Witness Release, and other forms as applicable— even a review and update of your cremation policies and procedures.
